None/Infrequent: 1 person 1 Very low/very unlikely 1 No, or minimal injury/illness/damage 2
None/Infrequent: More than 1 person 2 Low or unlikely 2 Minor injury/illness/disease/damage 4
Daily: 1 person
Daily: More than 1 person		 3
4		 Medium chance 3 Consequential illness/disease/damage 6
Frequent: 1 person 5 High or likely 4 Major injury/illness/disease/damage 8
Frequent: More than 1 person 6 Very high/certain 5 Fatality/disabling/intolerable damage 10

To calculate the ‘Risk Rating’, add the ‘Frequency, Likelihood, and Consequence’ scores. (E.G. 5 + 4 + 8 = a Risk Rating of 17 (High Risk)).
Score 1 to 11 = LOW RISK Score 12 to 16 = MEDIUM RISK Score 17 to 21 = HIGH RISK
